Important Things to Remember for 2011-2012
Rusk ISD Cafeteria plan year is September 1, 2011 through August 31, 2012.
MGM is offering the “BENNY” Card for ALL participants of the Medical Reimbursement Account this year. The MGM Flex Card that you currently have will expire on 8/31/2011. IF you have updated your address in the system, your new “Benny” Card will be sent to your home address 2 to 3 weeks after the first of September 2011. The fee for the flex card is $1.50 per month. Once you receive the Benny Card, do not throw it away. The card is good for 3 years and can be reloaded with your new election every year. These accounts are “USE IT OR LOSE IT.” Plan your contributions carefully! Always save your receipts. You will need them should you be audited or for documentation. Enrollment is required every year for the Flex Plans, as this benefit does not “rollover”.
The Humana/CompBenefits Dental plan will not have a rate increase this year. To view the rates, click on Benefit

Block Vision is the vision plan provider for 9/1/2011. The plan will have a small rate increase this year on the Family plan. To view the rates, click on Benefit Highlights / Vision.
The Unum Disability plan is the Educator Select Income Protection Plan. During the annual enrollment period, employees can increase or add disability coverage without providing evidence of insurability! New coverage amounts or changes are subject to pre-existing condition limitations. See plan information under the benefit highlight tab for details on plan options and benefit changes.
Hartford Life is the Voluntary Term Life, and Dependent Term Life carrier effective 9/1/2011. Amounts over $150,000 will require evidence of insurability applications. If electing coverage for the first time, evidence of insurability will be required. Electing coverage for your Spouse for the first time or coverage increases will require evidence of insurability.
Hartford is the carrier for AD&D coverage. You can purchase accidental death and dismemberment insurance in the amounts of $10,000 to $500,000 (maximum of 10x’s your annual salary). See plan information under the benefit highlight tab for details on plan options and benefit changes.
Plan offerings to Rusk ISD employees include: Humana/CompBenefits Dental Plan, Block Vision, Unum Educator Plan Disability, Philadelphia American (formerly CSO) Cancer, The Hartford Term Life and AD&D and Flexible Spending Accounts.
Please return any applications for coverage to Kay Gresham at the Rusk ISD Benefits Office. All benefits will be offered on a 9/1 effective date basis (pending applicable applications for life and cancer plans.) |
